Are employers required to make reasonable accommodations for your FMLA based on MD recommendations?

The question

My employer did not make accommodations, instead put me out of work completely. My 12 weeks are now up and my FMLA is ending, meaning my job will be posted, my benefits will be terminated. I will be having surgery within the next 2 months hopefully and now have to obtain COVRA Benefits at nearly $4500 a month
